Description
The SIBO (Small Intestinal Bacterial Overgrowth) Test by Genova Diagnostics is a non-invasive breath test used to detect abnormal bacterial growth in the small intestine.
After ingestion of a lactulose solution, breath samples are collected over time to measure hydrogen (H₂) and methane (CH₄) gases produced by bacterial fermentation.
👉 As shown in the report, gas levels are tracked across multiple time points and interpreted based on rise patterns
This allows identification of early fermentation in the small intestine, a hallmark of SIBO.
🔬 What This Test Measures
1. Hydrogen Gas (H₂)
- Produced by bacterial fermentation
👉 Rise ≥ 20 ppm within 90 minutes suggests SIBO
2. Methane Gas (CH₄)
- Produced by methanogenic organisms
👉 ≥ 10 ppm at any point indicates methane overgrowth
3. Gas Patterns Over Time
- Early rise → small intestine overgrowth
- Late rise → normal colonic fermentation
🔬 2 Hour vs 3 Hour Test
2-Hour Test
- Standard clinical test
- Focus on early detection
3-Hour Test
- Extended analysis
- Useful for slow transit or constipation
👉 Provides broader fermentation mapping
🧠 Clinical Insights
- Diagnoses SIBO
- Differentiates hydrogen vs methane dominance
- Identifies fermentation-related digestive issues
- Supports IBS evaluation
Methane-dominant overgrowth has been associated with constipation and slower gut motility
👤 Who Should Consider This Test
- Bloating after meals
- IBS symptoms
- Chronic gas
- Constipation or diarrhea
- Food intolerance
⚠️ Limitations
- ❌ Does not identify specific bacteria
- ❌ No microbiome composition
- ❌ No immune response measurement
👉 This test detects functional bacterial overgrowth only
🔷 Functional vs Immune Testing (Important Section)
| Feature | Genova SIBO | Cyrex Array 22 |
|---|---|---|
| Detects SIBO | ✔ Direct | ❌ Indirect |
| Measures | Gas production | Immune response |
| Sample | Breath | Blood |
| Best For | Active SIBO diagnosis | Chronic immune-related IBS |
👉 Use Genova to answer:
“Do I currently have SIBO?”
👉 Use Cyrex to answer:
“Has SIBO triggered immune damage?”
