Cyrex Array 22 — Irritable Bowel / SIBO Screen™
Many cases of IBS and SIBO are driven by post-infectious autoimmunity — molecular mimicry between bacterial toxins and gut nerve/muscle proteins. Array 22 identifies the autoimmune antibodies responsible for motility dysfunction and abnormal gut innervation, differentiating true autoimmune bowel disease from other causes of IBS-like symptoms.
Antigens Tested (IgG + IgA):
- Vinculin — cytoskeletal protein in the migrating motor complex (MMC); antibodies disrupt gut motility
- Cdtb (Cytolethal Distending Toxin B) — bacterial toxin from C. jejuni and other pathogens that triggers anti-vinculin autoimmunity
- CdtB/Vinculin complex antibodies
Why This Matters:
Dr Mark Pimentel’s research demonstrated that post-infectious IBS-D and SIBO with motility issues are driven by anti-CdtB and anti-vinculin antibodies. This is the only blood test that directly identifies this mechanism.

Array 22 – Irritable Bowel / SIBO Screen is a blood-based test that assesses immune activation triggered by bacterial overgrowth and toxin exposure in the gut.
Unlike breath tests that detect active bacterial fermentation, this test evaluates whether bacterial toxins have caused immune-mediated damage to intestinal tissues.
